Reminder: Transportation Requests

Dear Ottumwa Families, 

This is a friendly reminder about the opportunity to request transportation assistance for students residing in hazard zones around their homes. 

Reminder: 

Notification to Families: Parents may go to the District Bussing Map to determine if they are eligible for transportation.  Families may also review the following link on how to use the bussing map site. 

Hazard Zone Review Request: The request form will be available on our school district website and distributed along with the notification. Families who wish to have an area reviewed to be considered for a hazardous area may do so by completing this form

The safety and well-being of our students are our top priorities, and we want to ensure that all eligible students receive the transportation assistance they need to travel safely to and from school.

Q: How do I know if my family resides in a hazard zone? Parents may go to the District Bussing Map to determine if they are eligible for transportation.  Families may also review the following link on how to use the bussing map site. 

Q: What documentation is required to submit a Hazard Zone Request? A: Hazard Zone Request Form must be completed and submitted by the deadline on August 12, 2024, Additional documentation, such as proof of residency, may be required depending on the circumstances of your request.  Parents submit a request by completing this form
